Child Development Inc. in Tamaqua is holding an open enrollment on Tuesday, June 16, 2015 from 9 AM to 4 PM.  They are located at 307 Pine Street.
In order to be considered for enrollment, and Eligibility Application must be completed for your child(ren). If you have already completed an Eligibility Application and your child was not enrolled this year, you will need to complete a new Eligibility Application to be considered for enrollment this year.

Requirement is age 3 by September 1st.

The items that you will need to bring with you are:
– Proof of income (W-2, 1040, or 1040A or 1040EZ), one month’s pay stubs (weekly 4, biweekly 2), any other source of income you receive (child support, social security, etc.)
– Current immunization record for your child.
– Full address and phone number for 2 emergency contacts.
– Any court orders you have (child custody, child support, protection from abuse, etc.).
– Health insurance card.

The completion of eligibility should take no longer than 15 minutes.
