PRESS RELEASE – St. Luke’s University Health Network (SLUHN) is a nationally recognized non-profit health network that is composed of six hospitals and over 200 outpatient facilities serving counties in both Pennsylvania (Lehigh, Northampton, Carbon, Schuylkill, Bucks, Montgomery, Berks, Monroe) and New Jersey (Warren). As part of the Patient Protection and Affordable Care Act, all non-profit hospitals are required to conduct a Community Health Needs Assessment (CHNA) every three years. In order to accomplish our goals, St. Luke’s is conducting surveys to identify health needs within the community. Since you are a member of our community, you are being asked for your feedback to assist us in data collection. Your answers will be compiled by St. Luke’s to determine health needs in the community. This is an ANONYMOUS survey. You must be 18 years or older to complete this survey.
